Job
Stress Inventory The Job Stress Inventory (JSI) is an instrument designed to assess perceived levels and sources of stress within an organization. It is comprised of 57 multiple choice items and can be completed in under 20 minutes. By focusing upon perceived stress, the JSI provides critical information about how an individual's perceptions compare with those of the over 7000 employees in the normative sample. The JSI can be used for the measurement of occupational stress at the individual, group, and organizational level. The JSI is also suitable for use as a repeated measure in order to assess changes in perceived occupational stress over time. Customizations are available to support use of this instrument in a wide variety of group survey and research applications. Please contact us with your specific needs. The JSI assesses the following perceived workplace stressors: Physical Demands and Danger Uncertainty, Lack of Interpersonal Skills of Supervisor, Lack of Interpersonal Skills of Others, Time Pressure, Lack of Competence of Supervisor , Lack of Competence of Others, Lack of Control, Red Tape and Overall Stress. |

The Time Management Effectiveness Profile looks at the following competencies: Pre-disposition / Temperament, Preparation Skills, Organizational Ability, Pressure Management Skills, Delegation Skills, Interruption Management Skills, and Results Orientation. |

Stress Management Effectiveness Survey The Stress Management Effectiveness profile takes a holistic look at stress and well-being in the following competencies: Predisposition, Physical Fitness, Nutrition and Diet, Personal Well-Being, Emotional Balance, Emotional Habits and Control and Work Life. The report gives individuals a clear indication of their relative levels of stress, and detailed interpretative notes to use in personal action-planning. |

Stress Symptoms Checklist The Stress Symptoms Checklist (SSCL) is a 38 item self-report test designed specifically to assess anxiety-related stress symptoms, including symptoms of post traumatic stress disorder associated with chronic pain. People rate the frequency with which they have experienced each of the 38 symptoms during the past two weeks on a seven point scale ranging from one (never) to seven (always). Several scores are derived to include: Post Traumatic Anxiety Symptoms (PTAS), Pain and Impairment (PI), Total Sum of Ratings, Total Symptom Percentage. |

Life Style and Health Inventory
An effective way to begin working on reducing health risks in to take an inventory of health related behaviors. You will be asked to report certain data on lifestyle factors and health risks, necessary to evaluate your health status. The survey also asks for your height, weight, blood pressure and total cholesterol, so you will want to have these values in hand before you begin the inventory. This inventory will take you less than 10 minutes to complete. |

Health Status Questionnaire 2.0 The Health Status Questionnaire 2.0 (HSQ 2.0) is an outcomes measurement tool that yields a profile of scores on eight health attributes, an index of health status change, and an indication of risk for the presence of a depressive disorder. This instrument is designed to be easily administered via self-report, personal interview, or telephone interview. It captures aspects of both physical and emotional health including: Health Perception, Physical Functioning, Role Limitations / Physical health, Role Limitations / Emotional health, Social Functioning, Mental Health, Bodily Pain, and Energy and Fatigue. |

Quality of Life Inventory The Quality of Life Inventory (QOLI) assesses the importance of 16 quality of life dimensions to an individual and the degree to which the individual is satisfied or dissatisfied with each area in his or her own life. The areas include: health, self-esteem, goals and values, money, work, play, learning, creativity, helping, love, friends, children, relatives, home, neighborhood, and community. The QOLI takes 5 minutes to complete and it requires a 6th grade reading level. |

Integrating these 2 domains in 1 assessment tool provides a comprehensive picture of an individual's overall life context.
